By partnering with Forged Amputee Wellness, your practice acknowledges that Forged will collect information directly from patients who voluntarily enroll through the Forged website or other Forged registration methods. Forged will use this information to provide personalized wellness programming, educational resources, patient engagement services, progress tracking, quality improvement activities, and communications with enrolled participants.

With the patient's authorization, Forged may provide your practice with information related to patient enrollment, participation, exercise adherence, progress, and wellness outcomes to support care coordination and enhance the patient experience.

Forged Amputee Wellness will not sell your practice's information or use your practice's name, logo, or identifiable data for promotional purposes without your permission. Practice information and patient information will be maintained using reasonable administrative, technical, and physical safeguards and will only be used for purposes related to the services provided through the Forged partnership or as otherwise permitted or required by applicable law.
